About the Role

Join Southwest Ontario Aboriginal Health Access Centre as a Family Physician – $332K, HOOPP Pension, London ON in London, Ontario.

Key Responsibilities

  • Provide comprehensive primary care for SOAHAC clients, including health promotion, disease prevention, acute care, and chronic disease management.
  • Conduct assessments, develop care plans, provide follow
  • up, and coordinate ongoing primary care for SOAHAC clients
  • Collaborate with Nurse Practitioners and other interdisciplinary providers within their respective scopes of practice.

Requirements

  • Medical Degree (MD) and Certification in Family Medicine (CCFP), or equivalent.
  • Licence for Independent Practice, or eligibility, with the College of Physicians and Surgeons of Ontario (CPSO).
  • Experience working with Indigenous communities, community health centres, or underserved populations is an asset.
