Key Responsibilities
- Provision of comprehensive pharmaceutical care to patients
- Clinical care of patients in comprehensive clinical programs including warfarin dosing, IV to PO, aminoglycoside and vancomycin dosing.
- Use of Pharmacists' clinical scope of practice as per THP's Policy and Procedure to optimize patient care
- In collaboration with other clinical pharmacists deliver appropriate teaching to pharmacy residents and pharmacy students
Requirements
- Baccalaureate Degree in Pharmacy or Pharm D
- Licensed in Ontario to practice pharmacy
- Completion of hospital pharmacy residency or equivalent experience is an asset
- Strong interpersonal, problem solving, organizational, communication, documentation and critical thinking skills
Benefits & Compensation
- Hourly rate: $66.44/hr.
Work Schedule
- Full-time position.
- Weekend coverage may be required.
