About the Role
Join Advisa Care as a Per Diem/Part Time or Full Time Hospice MSW Social Worker in Flint, Michigan, United States.
Key Responsibilities
- Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
- Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record.
- Conducts reassessments of patient/family needs and counseling as required.
