Requirements

• Requirements Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.

• Kentucky Board of Pharmacy license is required.

• 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.

• Experience in ambulatory clinical pharmacy practice preferred.
