Registered Nurse - Remote Patient Monitoring - Home Telehealth Care Coordinator

About the Role

Join VA Greater Los Angeles Healthcare as a Registered Nurse for Remote Patient Monitoring in Home Telehealth. This role focuses on managing high-risk, complex patients using telehealth technologies to support Veterans in cost-effective, non-institutional settings.

Key Responsibilities

• Coordinate care delivery through Remote Patient Monitoring under the Home Telehealth program.
• Perform comprehensive clinical assessments and develop individualized care coordination plans.
• Triage biometric and subjective data from telehealth devices.
• Collaborate with interdisciplinary teams including mental health, pharmacy, and rehab services.
• Provide health coaching and disease-specific education to improve patient outcomes.
• Lead or participate in quality improvement initiatives.

Requirements

• Graduate of an accredited nursing program.
• Current, full, active, and unrestricted RN license in any US state or territory.
• Preferred 2-3 years RN experience and 2 years case management.

Benefits & Compensation

• Salary range: $75,000 - $95,000 per year.
• 50 days paid time off annually (26 annual leave, 13 sick leave, 11 federal holidays).

Work Schedule

• Monday through Friday, 8:00 AM to 4:30 PM.
