Key Responsibilities

• will support evidence-based practice, contribute to performance measurement and clinical audits, engage in continuous quality improvement activities, and help ensure the delivery of safe, effective, and patient-centered care in alignment with organizational standards and regulatory requirements.

• will do: Deliver high quality, patient-centered care in an FQHC look-a-like setting via in-person visits and telehealth.

• will bring: At a minimum MS in Nursing or other related field Current APN License in the state of Illinois Three years of clinical experience We are hoping for Bi-lingual language skills would be a great benefit to our patients.

Benefits & Compensation

• Salary: $110K - $135K

Work Schedule

• Full-Time position.

• Hybrid position - combination of remote and on-site.

• Call responsibilities required.
