Key Responsibilities

will be responsible for coordinating the care and services of select patients.

will intervene with individuals experiencing complex psychosocial and discharge planning challenges, deliver crisis intervention for patients and families and support safe, effective transitions of care for high-risk patients facing barriers to access.
