- 5 years clinical pharmacy experience in an ambulatory care setting under a Practitioner
- Pharmacist Collaborative practice agreement Completion of a PGY
- 2 in Ambulatory Care Pharmacy Board Certification issued by the BPS in Ambulatory Care Pharmacy (BCAPC)
