What You Should Know About the Social Worker Role: * The Social Worker would be for coverage under Ohio's Hospice South regions * This is a full-time position. M-F, 8A-5P with after hours/weekend on call * We provide superior care and superior services to patients at their end of life journey.

Key Responsibilities

• bereavement and anticipatory grief needs to offer a wide range of services to terminally

• ill patients and families.

Requirements

• Licensed Social Worker with the state of Ohio LISW preferred or LSW.

• Master's Degree in Social Work or related degree as required for LSW licensure.

• One year social worker experience in a health care setting preferred.

• previous discharge planning experience
