Join Ohio s Hospice Inc as a Medical Social Worker (FT Days) in United States. What You Should Know About the Social Worker Role: * The Social Worker would be for coverage under Ohio's Hospice South regions * This is a full-time position. M-F, 8A-5P with after hours/weekend on call * We provide superior care and superior services to patients at their end of life journey.
Key Responsibilities

Requirements
- Licensed Social Worker with the state of Ohio LISW preferred or LSW.
- Master's Degree in Social Work or related degree as required for LSW licensure.
- One year social worker experience in a health care setting preferred.
- previous discharge planning experience
