The Case Manager/Physical Therapist delivers 1:1 community-based neurorehabilitation and coordinates care plans with an interdisciplinary team. They are responsible for managing patient services, formulating treatment plans, and communicating progress to physicians and payers.
Candidates must hold a minimum of a Bachelor’s degree in an Allied Health field, Nursing, or Human Services and possess at least two years of experience with neurologically impaired patients. Current professional licensure and BLS/CPR certification are required, with CBIS certification preferred.
