Key Responsibilities

• Will Do Lead a patient-centered, pharmacist-led disease management program to optimize care for chronic diseases such as diabetes, COPD, and others, while promoting medication adherence and health education.

• Lead a patient-centered, pharmacist-led disease management program to optimize care for chronic diseases.

• Conduct Medication Therapy Management (MTM) and Comprehensive Medication Review (CMR) as well as other profe.
