About the Role

Key Responsibilities

  • Managing a caseload of adults requiring care and support, completing strengths‑based assessments, Mental Capacity Act assessments and developing person‑centred care plans.
  • Working collaboratively with health professionals, community services and safeguarding partners to coordinate interventions, reduce risk and promote independence.
  • Maintaining accurate case records, preparing professional reports and making evidence‑based recommendations for panels, best‑interest meetings and court where required.
