About the Role

Join Brio Living Services as a Physical Therapist – Per Diem, Flexible Schedule in United States.

Key Responsibilities

• Evaluates and assesses needs of referred patients and formulates treatment plans in a home care environment.

• Provides physical therapy services as defined in treatment plan and approved by physician.

• Serves as case manager when appropriate; makes referrals to other disciplines as indicated by the needs of the patient.

• Reports changes in the patient’s condition to physician and/or other health professionals when needed.

Requirements

• under the Federal Conditions of Participation for Home Health Agencies as established by CMS.

• Two (2+) years of service of Physical Therapy experience preferred.

• Must have excellent clinical assessment skills as they relate to Physical Therapy.

• Current Michigan Driver’s license and reliable transportation.
