About the Role

Join Interim HealthCare of the Twin Cities as a Hospice Social Worker – M-F Schedule, On-Call Rotation in United States. Medical Social Worker - Hospice - Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. The holistic approach encompasses physical, emotional and spiritual well-being.

Key Responsibilities

  • will have independence and flexibility in the field to visit patients one on one in their home environment.
  • will have a general territory within our service area where they will provide in-home hospice care to the patients.
  • Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families.
  • provide in-home hospice care to the patients.

Requirements

  • Hospice experience preferred
  • Able to travel within geographic area serviced by office. Valid drivers license, and proof of auto insurance are required. Some functions of what you would do as a Medical Social Worker
  • Conducts an initial psycho
  • Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.

  • Salary: $66K - $81K
  • CME allowance: $400.
  • 401(k) with employer match.

Work Schedule

  • Full-Time position.
  • Call responsibilities required.
  • Day shift only - no nights.
