About the Role

Join Interim HealthCare of the Twin Cities as a Hospice Social Worker – M-F Schedule, On-Call Rotation in United States. Medical Social Worker - Hospice - Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. The holistic approach encompasses physical, emotional and spiritual well-being.

Key Responsibilities

will have independence and flexibility in the field to visit patients one on one in their home environment.

will have a general territory within our service area where they will provide in-home hospice care to the patients.

Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families.

Requirements

Hospice experience preferred

Able to travel within geographic area serviced by office. Valid drivers license, and proof of auto insurance are required. Some functions of what you would do as a Medical Social Worker

Conducts an initial psycho

Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.

Benefits & Compensation

Salary: $66K - $81K

CME allowance: $400.

401(k) with employer match.

Work Schedule

Full-Time position.

Call responsibilities required.

Day shift only - no nights.
