Key Responsibilities

The Behavioral Health Clinician provides consultative behavioral health interventions and assessments within a primary care team to improve overall patient health. Responsibilities include delivering evidence-based interventions, creating care plans, and coordinating transitions to community partners.

Requirements

Candidates must hold a Master's degree in Social Work, Professional Counseling, or Family Therapy and possess a valid license (LCSW, LICSW, LPC, LMHC, or LMFT) in Oregon or Washington. A minimum of two years of clinical experience in a healthcare-related field is required.
