About the Role

Join Prologue Inc as a Rehabilitation Counselor - Direct Care in United States.

Key Responsibilities

and Duties Complete contact notes, Individual Service Plans, Monthly Progress Notes, and other documents as needed and/or assigned.

Assist individuals on goals and interventions as stated on the Individual Service Plan.

Provide day program coverage (on-site services), facilitate groups, and develop group lesson plans.

Carry caseload of individuals for rehabilitation.
