About the Role

Join VNS Health as a Senior Social Worker – Hospice Home Care in United States. Overview Provides clinical and consultative support to Licensed Social Workers (LSWs) and Interdisciplinary Care Team Managers regarding complex social work related clinical and/or customer service issues. Provides mentoring and leadership to less experienced LSWs.

Key Responsibilities

  • provide direct social work services to patients and families in collaboration with the interdisciplinary car.
  • Conduct field orientation and orientation co-visits for newly hired social workers.
  • Provide supplemental support, coaching, and clinical consultation as needed for both new hires and incumbent.
  • Conduct annual clinical co-visits for SWs within their assigned regions.
