About the Role
The PACT Social Worker is part of the Primary Care team at the VA Medical Center in Fayetteville, NC. The role focuses on prevention, health promotion, coordination, and chronic disease management for veterans.
Key Responsibilities
- Assess and treat complex psychosocial problems.
- Coordinate care with VA staff and community agencies.
- Establish therapeutic relationships with veterans and families.
Requirements
- Licensed Clinical Social Worker.
- Experience in primary care social work.

- Salary range: $88,163 to $114,606 per year.
