About the Role

Join Dialysis Care Center as a Social Worker in United States. PURPOSE AND SCOPE: Supports the mission, core values and customer service of Home Dialysis Services LLC (HDS). Adheres to the Home Dialysis Services Policies and Procedures and Compliance Program including all state and federal regulatory requirements.

Assist with home management in conjunction with the Inter

Communicate effectively with the IDT about patient issues, intervention and goals
