Join BAYADA Home Health Care as an Occupational Therapist performing home health visits for adult and geriatric patients in Somerset County, New Jersey. This per diem role involves working independently to provide rehabilitative care in patients' homes.

Key Responsibilities

• Conduct home visits to evaluate patients’ functional status including muscle function, endurance, and ADL/IADL performance.
• Assess cognitive, psychological, and motor skills to develop individualized intervention plans.
• Evaluate home environments for hazards and recommend equipment or adaptations.
• Collaborate with nursing and multidisciplinary teams to revise care plans.
• Educate patients and families on rehabilitative care and safety.
• Document all observations and interventions accurately using electronic medical records.

Requirements

• Current New Jersey Occupational Therapist license.
• Minimum one year recent OT experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently with strong organizational and interpersonal skills.
• Computer proficiency; EMR experience preferred.
