About the Role

Join Compassus as a Hospice Social Worker – MSW Required in United States.

Key Responsibilities

Assess patients’ psychosocial, financial, environmental, and community needs, as documented in the initial social work assessment, Plan of Care (POC), clinical records, and team meeting notes.

Provide direct and indirect social services support for patients and families in all service settings (homes, long

term care facilities, hospitals).

Deliver crisis intervention and supportive counseling for patients and families.

Requirements

California Social Worker License preferred upon hire.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions.

Adaptations can be made to accommodate team members with disabilities.

Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.
