Key Responsibilities

The physician provides direct patient care using evidence-based guidelines and a team-centered approach to manage chronic diseases and wellness. They are responsible for maintaining accurate patient records, coordinating transitions of care, and participating in process improvement initiatives to enhance access and quality.

Requirements

Candidates must hold a Doctorate (M.D. or D.O.) and be board certified or eligible. A valid Arizona medical license, BLS certification, and a Fingerprint Clearance Card are required for this role.
