Key Responsibilities
- Build trusting relationships and collaborate effectively with Black and racialized populations to identify and address community needs and barriers to access.
- Collaborate with the interdisciplinary team and external service providers to coordinate and manage client care, including chronic disease monitoring, episodic care, follow
- up, case management, and referrals.
- Facilitate timely referrals to specialists, hospitals, and community resources as required.
Requirements
- BScN with Nurse Practitioner certification; registered with the College of Nurses of Ontario in the Extended Class.
- based clinical nursing experience in primary health care or community
- based settings.
- Strong clinical judgment, assessment, and problem
