About the Role

Key Responsibilities
- Perform comprehensive health assessments and physical examinations during in-home visits and telehealth appointments.
- Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.
- Develop and implement individualized treatment plans tailored to each patient’s needs.
- Educate patients and families on health management, wellness, and disease prevention.
Requirements
- based care experience strongly preferred. Experience caring for patients with disabilities is highly desirable. Comfortable delivering care in both in
- home and telehealth settings. Excellent communication skills and a collaborative, team
