Key Responsibilities

Completes an initial assessment of patient and family to determine home care needs.

Provides a complete physical assessment and history of current and previous ill ness(es).

Regularly re-evaluates patient nursing needs.

Initiates the plan of care and makes necessary revisions as patient status and needs change.

Requirements

Current Registered Nurse with NYS Department of Education required.

1 year of medical/surgical nursing experience required.

(MRDD and/or prior home care experience preferred) New grads welcomed, experience a plus.

Associate or Baccalaureate Degree in Nursing from an approved program preferred.

Work Schedule

Full-time position.
