Key Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.

• Provides a complete physical assessment and history of current and previous ill ness(es).

• Regularly re-evaluates patient nursing needs.

• Initiates the plan of care and makes necessary revisions as patient status and needs change.

Requirements

• Current Registered Nurse with NYS Department of Education required.

• 1 year of medical/surgical nursing experience required.

• (MRDD and/or prior home care experience preferred) New grads welcomed, experience a plus.

• Associate or Baccalaureate Degree in Nursing from an approved program preferred.

Work Schedule

• Full-time position.
