About the Role
Join DaVita IKC as a Nurse Practitioner providing hybrid remote care in Lancaster, Pennsylvania. You will manage complex kidney patients with chronic illnesses, delivering holistic, integrated care in collaboration with nephrologists and interdisciplinary teams.
Key Responsibilities
- Complete Comprehensive Health Evaluations to assess and manage comorbid conditions.
- Manage CKD and ESKD patients focusing on slowing disease progression and reducing hospitalizations.
- Lead clinical teams to ensure coordinated care addressing medical and psychosocial needs.
Requirements
- Minimum 2 years' experience as a Nurse Practitioner.
- Current unrestricted NP license and DEA license or eligibility.
- CPR/BLS certification.
- Strong clinical judgment and autonomous decision-making.

- Salary range $84,000 - $133,000 per year.
