About the Role
Join Healthforce as a Travel Social Worker – 13-Week Contract in United States.
Key Responsibilities
- Interview patients or designees to gather psycho
- social information and assess the impact of illness on patients and families.
- Participate in the interdisciplinary care planning process and coordinate the discharge planning and referrals for home social services, equipment, or supplies.
- Provide therapeutic support and education to patients, residents, and families; help interpret medical recommendations; refer for additional services as needed.
Requirements
- Licensure: Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) with Alaska licensure at time of submittal.
- Certifications/clearances: Flu vaccine required; NABCS Clearance; ICPA Clearance; two fingerprint cards required upon acceptance of contract.
- Experience: Minimum 2–4 years of experience in an inpatient or outpatient healthcare social work setting.
- Age: Must be at least 21 years of age.

- Salary: $142K - $150K
