Key Responsibilities

• will be actively engaged in all complexities of patient medication management, monitoring, and education, as needed.

• lead Chronic Disease Management in Primary Care using broad CPAs.

Requirements

• specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as needed. May design a drug therapy plan for patient

• related treatment plans, assessment of drug therapy, outcomes and identification and resolution of drug

• Illinois Department of Financial and Professional Regulation (IDFPR) And Board Certified Ambulatory Care Pharmacist (BCACP)

• Board of Pharmacy Specialties (BPS) Or Board Certified Cardiology Pharmacist (BCCP)
