The Behavioral Health Case Manager provides therapeutic services, develops individualized treatment plans, and coordinates care for clients. They also facilitate linkage to resources, perform crisis prevention, and maintain positive relationships with external contacts.
Candidates must be at least 18 years old with a high school diploma and three years of experience, or hold a Bachelor's or Master's degree in a related field. A valid driver's license and reliable transportation are required for travel within the service area.
