About This Hospice Registered Nurse (RN) Case Manager Position HopeHealth Hospice & Palliative Care is seeking an experienced RN to care for hospice patients in long-term care facilities in Northern Rhode Island. This is a full-time, 40hrs per week, position with an 8am-4:30pm schedule.
This position offers weekends off, day shifts only. Work with HopeHealth in Providence, Rhode Island.
The Registered Nurse Case Manager conducts initial and ongoing patient assessments while collaborating with physicians to manage and evaluate individualized plans of care. They also provide education to families, coordinate community services, and maintain accurate clinical documentation regarding patient progress.
Candidates must be graduates of an accredited nursing school with a valid RN license in Rhode Island and Massachusetts. A minimum of 1-2 years of clinical experience and strong assessment and communication skills are required.
