About the Role

Join Sequoia Home Health as a Medical Social Worker (MSW) – Hospice in United States.

Key Responsibilities

Conduct comprehensive social work evaluations and develop individualized care plans.

Provide emotional support, counseling, and crisis intervention to patients, caregivers, and families.

Deliver services ordered by the physician and outlined in the hospice plan of care.

Communicate regularly with physicians, RNs, and interdisciplinary team members.

Requirements

Master’s degree in Social Work (MSW/DSW/PhD) from a CSWE‑accredited school.

1+ year of social work experience in a healthcare setting (hospice experience preferred).

Current CPR certification.

Valid driver’s license, reliable insured vehicle, and ability to travel within assigned service areas.

Benefits & Compensation

Hourly rate: $50/hr.
