The Physical Therapist will evaluate, treat, and monitor home health patients to restore function and manage pain within their place of residence. They are responsible for collaborating with the interdisciplinary team to develop care plans and maintaining accurate, timely clinical documentation.
Candidates must hold a valid professional Physical Therapy license and have graduated from an accredited Physical Therapy program. Prior experience in home health is preferred.
