Key Responsibilities
- Performs comprehensive patient medication order evaluation, check for problems with dosages, interactions, and incompatibilities
- Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols
- Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes as necessary
- Assist with pharmacokinetics consult services and drug dosing per organizational protocols
Requirements
- Performance demonstrates efforts to improve patient satisfaction, lower costs, improve quality, and promote safety
- Is punctual and dependable; reports to work as scheduled; fulfills on
- call obligations per prearranged schedule; absenteeism and tardiness are within policy guidelines
- Creates a sustainable rounding schedule to provide clinical pharmacy services to patients as part of the medical team
