About the Role

Join Eden Health as a Hospice & Palliative Care NP – Autonomy in United States.

Key Responsibilities

will work autonomously but in collaboration with Physicians to provide primary medical care to patients with chronic or life limiting illnesses that require Palliative or Hospice care interventions.

will be responsible for rounding in hospitals and skilled nursing facilities, reviewing patients currently on our services and updating care plans accordingly.

will assist with advanced directives and discuss goals of care and the disease process, progression, and grief process with patients and caregivers.

will work with the Director of Patient Care Services and Executive Director to provide education on Palliative and Hospice care within the community as well as agency staff regularly.

Requirements

Current license from the state Board of Registered Nursing

Master’s Degree from an accredited Nurse Practitioner’s Program

National Certification as a Nurse Practitioner or certification eligible. Certification must be obtained within the first six (6) months of employment

A minimum of one (1) year Nurse Practitioner experience preferred
