About the Role
Join Eden Health as a Hospice & Palliative Care NP – Autonomy in United States.
Key Responsibilities
- will work autonomously but in collaboration with Physicians to provide primary medical care to patients with chronic or life limiting illnesses that require Palliative or Hospice care interventions.
- will be responsible for rounding in hospitals and skilled nursing facilities, reviewing patients currently on our services and updating care plans accordingly.
- will assist with advanced directives and discuss goals of care and the disease process, progression, and grief process with patients and caregivers.
- will work with the Director of Patient Care Services and Executive Director to provide education on Palliative and Hospice care within the community as well as agency staff regularly.
Requirements
- Current license from the state Board of Registered Nursing
- Master’s Degree from an accredited Nurse Practitioner’s Program
- National Certification as a Nurse Practitioner or certification eligible. Certification must be obtained within the first six (6) months of employment
- A minimum of one (1) year Nurse Practitioner experience preferred
