Key Responsibilities

  • face assessment services to patients to assist the Medical Director and team to determine each patient’s continuing eligibility for palliative care.
  • Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility.
  • Establishes a plan of care for continued needs based on the client assessment.
  • Provides patient care education, counseling, medication advice, instructions and follow
  • Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.
  • Assists with the development of educational programs in the palliative care program for diverse age groups.

Requirements

  • Tuition reimbursement for degree
  • Currently licensed to practice in the state serviced, with Masters in nursing.
  • At least 1 year of nursing experience
  • Currently licensed to practice in the state serviced, with Masters in nursing
