- Facilitate the development, implementation, and ongoing review of patient treatment and discharge plans through active participation in interdisciplinary team meetings.
- Complete case management duties for assigned patients, including preparing all required documentation, discharge materials, and psychosocial assessments within established timeframes.
- Initiate discharge planning at the time of admission and maintain consistent documentation of all discharge‑related activities in the patient record.
- Demonstrate behaviors aligned with SH Service Excellence standards by greeting others warmly, treating everyone with dignity and respect, and serving as a positive ambassador for Susquehanna Health.
