Key Responsibilities
- Delivers varied social work services to Hospice patients and their families.
- To provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services, and community education, outreach, and referral.
- The Hospice social worker is an integral part of the Hospice IDG.
