Job Summary The Social Worker LMSW (Licensed Master Social Worker) will provide generalist social work services across various care settings. This role involves assessing patient or outpatients’ needs, developing treatment plans, and providing therapeutic interventions.
Key Responsibilities
- Patient Assessment & Evaluation: Conduct thorough psychosocial assessments to understand patients' mental health, social, and environmental needs.
- Identify and evaluate patients’ strengths, challenges, areas requiring intervention and barriers to effective care.
- Makes recommendations to provider for treatment options and dispositioning.
- Treatment Planning & Intervention: Develop and implement individualized treatment plans in collaboration with LCSW/provider patients, families, and the healthcare team.
