Key Responsibilities

Assist with facilitation of rehabilitation of patients with emotional, physical or mental disabilities

Assist with implementation of physical therapy care plans

Assist with coordination of care with entire care team

Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members Must Haves

Requirements

Physical Therapist Assistant in the State of Texas

Skilled Nursing, Healthcare (Long Term Care) or Veterans Home experience highly preferred

Knowledge of Home Health regulations and general healthcare compliance

Experience with EMR

Work Schedule

Per Diem position.

No explicit call, weekend, or night shift information provided.
