About the Role

Join Trinity Hospice as a Social Worker in United States. Job Summary: The Social Worker will provide social services under the direction of a Physician, to offer support to our patients and their families. Social work services will be based on the patient’s psychosocial assessment, and the patient and family’s needs.

Requirements

of the agency/licensing board.

Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.

Promote Agency philosophy and administrative policies to ensure quality of care.

Attend interdisciplinary team meetings, participate in the patient care planning process and collaborate with the Interdisciplinary Team to promote coordination of patient care.

Work Schedule

Full-time position.

Call responsibilities required.
