- will collaborate closely with our interdisciplinary team, including nurses, therapists, and administrative staff, to provide individualized care and ensure high-quality patient outcomes.

Quality-of-Life & Compensation Flexible schedules to promote work/life balance Competitive pay: $72–$81/hour DOE or $150,000–$170,000 annually depending on experience Bonuses based on quality metrics, not patient volume Paid Time Off: up to 10 days annually, with clear accrual and usage policies Vehicle reimbursement or option for a company vehicle (based on availability) Comprehensive Benefits Medical, Dental, Vision Life Insurance 401(k) with employer matching Supportive and collaborative team environment Responsibilities Provide physical therapy services within the scope of practice, following individualized plans of care Deliver treatment, evaluate patient response, and report changes to the Physician, Case Manager, and/or Director of Nursing Maintain appropriate clinical records and submit documentation on company devices per organizational policy Follow all company policies and procedures Perform other duties as assigned Requirements Current California PT License (required) Valid California Driver’s License Experience: Minimum 2 years of Home Health experience required.

Requirements
- Time / Per Diem Schedule: Flexible. Monday – Friday standard, with rotational weekend coverage (1 weekend per month/quarter) required. Annual Full
- Time Expectation: $150,000 – $170,000 per year + $10K Sign on BONUS Pay: $72.00 – $81.00 per hour + $10K Sign on BONUS This is NOT a contracted position
- 00 – $81.00 per hour + $10K Sign on BONUS This is NOT a contracted position
