Key Responsibilities

  • The Licensed Program Case Manager provides supportive case management services to individuals with mental illness in the apartment treatment program.
  • Monitors maintenance of sites and provides on-site visits to scattered housing locations.
  • Plan and evaluate service plans and monitor objectives.
  • Write progress notes on service plan weekly.

  • Qualifying experience: Verifiable full or part time case management or case work with persons with a history of mental illness, homelessness, or chemical dependence. Our
