About the Role
Join Texas Health Resources as a Maternal Health Social Worker in United States. Maternal Health Social Worker Bring your passion to Texas Health so we are Better + Together Work location: Texas Health Resources, 612 E. Lamar, Arlington, TX 76011 Work hours: Full-time, 8:00am – 5:00pm with flexibility requirements with some travel.
Key Responsibilities
- Will Do: Care Coordination/Navigation: · Coordinate with NICU, Obstetric Emergency Nurse, Labor and Delivery Triage nurses, social workers, care transition manager, community health workers to identify patients with post-discharge needs · Assist with ensuring discharge instructions are followed by assisting patient with resources to comply with them.
- Must navigate patients to community resources and ensure connection was made and needs were met.
- Coordinate with NICU, Obstetric Emergency Nurse, Labor and Delivery Triage nurses, social workers, care transit.
- Conduct non-clinical psychosocial screening to identify social needs, safety concerns, and barriers affectin.
