About the Role

Join Texas Health Resources as a Maternal Health Social Worker in United States. Maternal Health Social Worker Bring your passion to Texas Health so we are Better + Together Work location: Texas Health Resources, 612 E. Lamar, Arlington, TX 76011 Work hours: Full-time, 8:00am – 5:00pm with flexibility requirements with some travel.

Key Responsibilities

Will Do: Care Coordination/Navigation: · Coordinate with NICU, Obstetric Emergency Nurse, Labor and Delivery Triage nurses, social workers, care transition manager, community health workers to identify patients with post-discharge needs · Assist with ensuring discharge instructions are followed by assisting patient with resources to comply with them.

Must navigate patients to community resources and ensure connection was made and needs were met.

Coordinate with NICU, Obstetric Emergency Nurse, Labor and Delivery Triage nurses, social workers, care transit.

Conduct non-clinical psychosocial screening to identify social needs, safety concerns, and barriers affectin.
