About the Role

Join Signify Health as a Nurse Practitioner for In-home Visits in United States. Key Responsibilities

will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

Conduct in-home visits that focus on a person’s overall health and well-being and identify potential barrier.

Benefits & Compensation

Salary: $80/assessment

Malpractice insurance covered.

Work Schedule

Contract position.

Weekend coverage may be required.

Day shift only - no nights.

Flexible scheduling available.

Requirements
