- Unrestricted Arizona Nurse Practitioner OR Physician Assistant license or ability to obtain prior to start date
- National Nurse Practitioner certification through AANC or AANP; OR Physician Assistant national certification through NCCPA
- Current DEA registration or ability to obtain prior to start date
