About the Role
Join HealthPartners as a Physical Therapist – Home Care, Occasional Weekends in United States. Park Nicollet is looking to hire a Physical Therapist to join our Home Care/Hospice team! Key Responsibilities
- will communicate in a timely, clearly, and professional manner with staff, patients, patient family members, physician, and other internal/ external customers.
- will require proficiency with communication and data processing equipment, clinical apparatus related to patient care in homecare, electronic medical record system, OASIS evaluation and data set Preferred Qualifications: * Work experience as a Physical Therapist for at least 3 years preferred.
- develop a comprehensive plan of care.
- provide appropriate service regarding primary diagnosis and multiple comorbidities affecting care.
Requirements
- Education, Experience or Equivalent Combination:
- Bachelors, Masters, or Doctorate degree in Physical Therapy
- Licensure/ Registration/ Certification:
- Current license with Minnesota Board of Physical Therapy
Benefits & Compensation
- Salary: $82,638 - $123,947/year
Work Schedule
- Full-Time position.
- Weekend coverage may be required.
