About the Role

Join HealthPartners as a Physical Therapist – Home Care, Occasional Weekends in United States. Park Nicollet is looking to hire a Physical Therapist to join our Home Care/Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Key Responsibilities

  • will communicate in a timely, clearly, and professional manner with staff, patients, patient family members, physician, and other internal/ external customers.
  • will require proficiency with communication and data processing equipment, clinical apparatus related to patient care in homecare, electronic medical record system, OASIS evaluation and data set Preferred Qualifications: * Work experience as a Physical Therapist for at least 3 years preferred.
  • develop a comprehensive plan of care.
  • provide appropriate service regarding primary diagnosis and multiple comorbidities affecting care.

Requirements

  • Education, Experience or Equivalent Combination:
  • Bachelors, Masters, or Doctorate degree in Physical Therapy
  • Licensure/ Registration/ Certification:
  • Current license with Minnesota Board of Physical Therapy

Work Schedule

  • Full-Time position.
  • Weekend coverage may be required.
