EDUCATION, EXPERIENCE, TRAINING 1.

Minimum of one years’ experience working in case management, discharge planning or other health related field preferred.

Working with persons with or at high risk of HIV infection preferred.

High School degree required; Associate degree or higher in Social Work, psychology or other health care related field preferred.
