Key Responsibilities
- Conduct comprehensive nutritional assessments of participants to identify their dietary needs and preferences across the continuum, including occasionally visiting home and community settings.
- Actively participate in the IDT by offering clinical expertise in team planning, problem-solving, and processes.
- Develop and implement individualized nutrition care plans to address participants’ health conditions and dietary goals.
- Assess nutritional needs, diet restrictions, and current health plans to develop and implement dietary care plans and provide nutritional counseling.
Requirements
- Serve on various internal and external committees as required or designated.
- Participate in team meetings, caregiver groups, and special events as needed.
- Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
