About the Role
Join New England Life Care as a Clinical Pharmacist in United States. New England Life Care (NELC) is one of the fastest growing home infusion therapy services companies in New England and is the region’s only non-profit home infusion provider. NELC is a hospital collaborative serving more than 70 hospital systems in Maine, New Hampshire, and Massachusetts.
Key Responsibilities
- Reviews prescriptions issued by physicians / authorized prescribers to assure accuracy and determine formulas and ingredients needed for dispensing the order.
- Collaboratively assesses the suitability of each patient for home infusion therapy.
- Assists the referral source in developing and implementing a plan for each patient’s smooth transition from hospital to home.
- Develops, implements, and monitors patient’s individualized plan of care to include setting of therapeutic goals, monitoring of progress to goals and ongoing communication with prescribing physician.
Requirements
- Demonstrates competency in the use and management of medication administration equipment.
- Ensures that confidentiality of patient information is maintained in accordance with state and HIPAA Privacy and Security regulations. Educational / Professional Requirements:
- Bachelor of Science degree from an accredited school of pharmacy.
- Registered Pharmacist, in good standing, with a valid license. Skills:
