NELC is a hospital collaborative serving more than 70 hospital systems in Maine, New Hampshire, and Massachusetts.

Key Responsibilities

• Reviews prescriptions issued by physicians / authorized prescribers to assure accuracy and determine formulas and ingredients needed for dispensing the order.

• Collaboratively assesses the suitability of each patient for home infusion therapy.

• Assists the referral source in developing and implementing a plan for each patient’s smooth transition from hospital to home.

• Develops, implements, and monitors patient’s individualized plan of care to include setting of therapeutic goals, monitoring of progress to goals and ongoing communication with prescribing physician.

Requirements

• Demonstrates competency in the use and management of medication administration equipment.

• Ensures that confidentiality of patient information is maintained in accordance with state and HIPAA Privacy and Security regulations. Educational / Professional Requirements:

• Bachelor of Science degree from an accredited school of pharmacy.

• Registered Pharmacist, in good standing, with a valid license. Skills:

• Hourly rate: $61/hr.
