Key Responsibilities

• include, but are not limited to: Provides direct patient care, evaluates functional needs and outcomes, and establishes plan of care; also evaluates the efficacy of treatment, modifying if necessary.

• Conducts all therapy practice within defined standards of care.

• Documents evaluations, treatment goals, and plans in accordance with Hospital.

Requirements

• and established guidelines.

• Regularly updates documentation and maintains prescriptions and signature.

• Doing so in a timely manner Coordinates, delegates, and supervises responsibilities assigned to support staff.

• Coordinates with interdisciplinary team on patient needs, treatment modifications, and progress including preparing plan for discharge.
